Insert the default into the DB when you create the employee record.
Me? I'd keep a separate table of images which includes the default, and use a foreign key to access the image data via a JOIN.

But ... is it a good idea to use two images? It could very easily be seen as sexist, or genderist if LGBT employees are involved. I'd stick to one single "no image available" marker myself, without a sex / gender association.
