I need to import data from CSV into the Cosmos DB using the migration tool.  How do I determine what is need in the connection string per the documentation below?  I know that database name but not sure what the two end points need to be.  

AccountEndpoint=<DocumentDB Endpoint>;AccountKey=<DocumentDB Key>;Database=<DocumentDB Database>;
