Sorry to hear you are suffering from this issue. Since I don't have the detailed log of error, I can not give exactly reason. But from my experience, I think it most likely there are some deployments which has the same name with the new deployment you are trying to add. Could you please check this? You can rename your deployment or delete previous failed deployments would solve this issue.
