This worked successfully for me and I'm using VS 2010.  I'd have to try this with just VS 2008 installed.  Of note, I have both installed, but it used 2010 when it pulled up the project.

As a workaround, you can create the application (which just generates the Cert and Application ID) and then use the Hello World Sample that comes with the SDK.  C:\Program Files (x86)\Microsoft HealthVault\SDK\DotNet\WebSamples\HelloWorld
