We have an application that we sell to doctor’s offices. We have written an interface into the HealthVault so we can upload CCR files and make them available to the patients. We are working through the go live steps and are a little confused on the privacy and term of use items.

Our application is built as a master/child app so each office can upload independent of each other. We are using the drop-off/pick-up method. The application allows the doctors office to create a child app (one time process) and in doing so, they must provide a privacy policy and terms of use. Are there any examples for these items? We would like to provide doctors with a template if possible. Also, what would we put in our privacy policy and terms of use since we believe no one would ever see those but us.
