Card holder''s name, card number (16), expiry date (mm/yy) and security code (xxx) would be enought in most cases.
For example, some other card vendors, uses different field lengths:

• Solo or Switch to Maestro uses 19 digits for card number
• Laser uses 18 digits for card number
• Amex cards uses 4 digits for CVC
• some cards has "issue date", which has the same purpose as "CVC" field
