I have a SharePoint 2010 list with a new item form that has a people picker field.  One of my users is having an issue that no one else seems to have.  When he opens the address book he finds his name and double clicks it but it is not populating in the form.  Any ideas?

From your description, Only one user can find his name in People Picker Modal Window, but when he double clicks it but it is not populating in the form. 

Does the special user can find other users and populate other users in the form?

Does the special user can populating his name in the form using Check Names button?

Does other users can find the special users and populate the special users in the form?
