I am creating a web app using Access 2013.

Over night it appears that the predefined buttons for Save, Delete, add etc have changed their icons, the buttons still work as they should.

How do I get the correct icons back?

Hi Peter,

Could you try something for me and let me know if this fixes the problem for you?

1. Change the SharePoint site theme where this app is hosted back to the default Office SharePoint site theme.

2. Save those changes.

3. Reapply the SharePoint site theme that you were previously using.

4. Save those changes.

5. Navigate to your Access web app in that site and see if the Action Bar icons are back to what thy should be.

If you do not understand any of these instructions, let me know. Also, please let me know if this fixes the issue for you.
