Is there code that will store certain values of a continuous form to a new table? Or to have a procedure that creates a table with specified fields from the form?

Well if you bound the Continuous Form to a Table/Query, there is no code needed.

You can do this by go to Form properties Tab Data, and then go to the Recordsoure and select the Table/Query.

Any specific reason you want to have this not bound to a Table/Query?
