I am new to Sharepoint. I have a list of 100 applications with the respective owners which I need to validate if the applications are in use. I need to create a form/list/survey in Sharepoint 2010, where I display the list of applications and the owners with a checkbox next to each application. The owner has to check box if the application is still in use and save and if it is not being used then save without checking.

Here are suggestions for your reference, create a master list(100 applications), then create another list for ‘survey’ with a Lookup filed reference master list.
