We're having an issue where an old TFS farm solution reference is re-added to our web app's web.config in a semi random fashion.  It's semi random in that these entries to web.config appear to be related to completing other SharePoint setup tasks (like installing PowerShell for SharePoint or something like that).  Entries like the following are re-added to our web.config causing the whole web app to die:
