Hi all

i have been looking around for a form that can submit data into the database and display a result , i need an example of a form like , select a phone and a model then , if a customer select a type of item , let''s say a model or brand of a phone , then click on submit , it automatically display a result of how much it cost from the database . can somebody help me ?
