嗨 我想将图片上传到表单并发送到数据库，我希望用户可以上传多张图片并同时发送到DB.此表单还具有标题和描述，任何机构都可以给我打电话，告诉我如何制作表单或给我看一些样品，

Hi I wanna upload picture to my form and send it to my DB,I want user can upload several pictures and send it in same time to DB.This form have also Title and description ,Any body can tel me how can i make my form or show me some sample,
