It appears the APIM admin portal UI for adding Custom Domains has changed, and there is no apparent way to enter the custom Hostname for the custom Gateway (formerly Proxy, I think) URL.  The situation is similar when attempting to configure the Custom Domain for Portal (Developer Portal).

The Hostname text box actually appeared when we tried again later, so we were able to complete the configuration of custom domain hostnames for Gateway and Portal.
