Hello,

I am currently creating a Task Manager App in SharePoint in order to allow others in my department to submit a task for an issue for me to resolve and have them be able to check on the status as well. I'm getting stuck on creating the form. I have many different categories that the issue could be under and depending on what category they choose I would like to have a different set of fields for them to fill out pop up. Is anyone familiar with how to do this.

Example: They have a claims inquiry and I need the provider and date of service so those fields would pop up as soon as they choose claims under the category. Whereas if they had a benefits inquiry I would only need the members name, plan information, and ID # so the provider and date of service field should not pop up when they choose benefits under category.

Any help would be much appreciated.
