当我进入表单设置时，我可以看到选项单选按钮被选中

重新发布表格后。但当我去打开表单时。它会进行一些重定向，然后出现一个空白页:(

after republishing the form. But when i go and open the form.It does some redirection and then a blankpage appears :(
When i go back to the form settings it set it back to the option - Use the default SharePoint form

Any ideas what could be wrong?
