We're encountering an issue with alerts/emails for Code Reviews.

For at least one person, but maybe not all, when they initiate a code review and the reviewer responds, they do not receive an email alert saying that the reviewer has responded and therefore don't know that they have.

Here's what we've confirmed/checked:

The person in question receives other email alerts from TFS correctly such as build emails, release emails, emails for assigned work items and so forth.  So their profile and associated email address are not an issue.  And since the server sends email successfully it is not an issue with the email server settings and so forth.

There is a team alert in the team project for "A code review I am working on changes".  It's the default you get when you add an alert and select that choice with an additional filter of team project = [current]

Other code review alerts/emails are generated.  It is just the ones related to a "Code Review Response" that appear to not be sent.

This TFS server was upgraded in place from TFS 2013 (and before that TFS 2012, 2010 and so forth).

I'm at a loss as to what else to check that would explain this issue.

Please check whether other team projects have the same issue. You may try to re-add these users to the team project group(s), then check the result.
