We recently migrated to Azure a very small number of users are being asked to authenticate by mobile. This is happening to 5 users so far (out of 300), but that number appears to be growing by 1 per week! Nobody in IT is doing anything to these users to enable!

But in the Azure Sign-In log it shows MFA Required: Yes, which the user actually receives!

Are you using Conditional Access? Can it be that you have a policy that requires MFA?
