We currently have a number of sites hosted across 9 instances in Azure App Services (Premium Service Plan - P3v2).  From what I've gathered, the built in load balancer in App Services is pretty basic.  No site health checks, etc.  Is it possible to use Azure Load Balancer to get more granular control over load balancing instead of using the default one?  If not, what about Application Gateway?

Right now creating another load balancer in front of your Web Apps isn't supported due to the networking setup of the Web Apps themselves. However, you could look at something like Traffic Manager.

What's your goal with another load balancer? An application gateway would definitely be a good option. It'll allow you to do SSL offloading also, along with some security features.
