I've got a strange situation where I can connect to my SharePoint Online site using the browser and via SharePoint Designer.  I can add, edit and publish workflows across all lists and libraries in my site with SharePoint Designer, but when I try and create a  custom form, I'm getting an error 'could not save the list changes to the server'.
