You don't just grab any smartcard and start coding it, and normally you won't need or want to mess with the internals of the card itself.

What you are probably needing to look at is interfacing the smartcard with your systems, which is likely to use USB hardware, and then VB will probably work. I'd start by looking at the smartcard you are going to be using and the connection hardware it requires and talk to the manufacturer about how the interface is presented to your software. It may be they have an API or .NET assembly to do the "donkey work" for you and your software only needs to interface with that.
