Use a more specific pattern than .* like [^/]+ and use /?$ to make the trailing slash optional:

RewriteRule ^user/([^/]+)/?$ profile.php?user=$1 [L]
RewriteRule ^user/([^/]+)/([^/]+)/?$ profile.php?user=$1&v=$2 [L]
