I'd strongly suggest you don't.
Partly because emails aren't meant to be interactive - and many spam filters will block interactive features to prevent phishing ans such like - and partly becuase it doesn;t work well in all email clients anyway.
Have a look here: Using Forms in Email: Method or Madness?[^] it shows a wayto do it, but also shows some of the places it doesn't work...
Direct them to an HTML page instead, it's a lot friendlier!
