we have a large document library in SharePoint 2013 Standard. One set of documents may be viewed by everyone. Some documents should only be visible for a limited number of users. We do not want to set the permission on item level because we think this requires to much effort and is error prone.

You could either use a separate document library (having more restrictive permissions) or a folder within the actual document library (you have to break the permissions' inheritance for that folder, of course).
A factor that could drive the choice of the solution would be the number of documents: if you expect a huge number of "confidential" documents, a separate library would be the best solution.
