We have two web apps and they are configured to only allow certain IPs access (everything else should be blocked).

One of them appears to be blocking an IP address that is in the 'approved' list and displays the following page:

They are configured exactly the same so how come I'm seeing different pages when taking the same action? What should the correct output be when blocking an IP?

If they both have the same set of ipsec rules, they should behave exactly the same.   Could you share the site name (one that works and one that doesn't) here or indirectly?   Also provide the UTC time when you hit 403 (this helps us scope where to look for issues).
