What is the expected (or worse case) between call to GetSystemTimePreciseAsFileTime on Azure vs a local PC with a GPS. We will be running part of our simulation on Azure and on pc and need the UTC time to be synchronize / precise.

The document you found is the best information we have available for the time sync in Azure. I am not aware of a way to decrease the possible offset to less than 1ms. The time sync is handled by the platform which you have no control over. That being said, as the article states many things depend on this time sync so we do our best to keep it as in sync to the best of our ability. In my experience there is nothing more that can be done to decrease the possible offset.

Each application would act differently during this update. From a customer perspective, these updates happen and the users never have any idea. They happen very quickly and without disruption of the services running on the VM. The platform will correct the time it was paused for but that could be an interesting thing to have when working with GPS.
