I am using office 365 with sharepoint and the forms app.

I have a list in sharepoint with a hyperlink to a form in a column. I would like it to then open the form and once it is completed attach the results as either an attached file or data to that particular list item.

eg i need to complete the same form once a month, hence by the end of the year there will be 12 copies of the same form.

Just to understand why you need a list to put a link to the form.

You can create forms and write Microsoft Flows for corresponding form which allows you to store form data in a list.

So eg, if you fill a form every month, at the end of year you will have 12 items in a list and each item correspond to one form data.

Then you can put link to these forms anywhere on your SharePoint page or you can embed the forms itself.
