My current version of tfs is 2015 update 1. There are 2 load balanced app tier servers, and SQL server reporting services are configured. I now want to upgrade to tfs 2015 update 4. I'm trying to find some detailed documentation/some kind of checklist of everything I need to do for this. Is it as simple as backing up all the databases, and then running the update 4 .exe on one of the app tier servers?

You need to stop services on secondary ATs. You need to do this before you start upgrade on primary AT. This is needed to ensure that secondary ATs are not trying to serve user requests/execute jobs. Let say you have 2 ATs - AT1, AT2. The sequence should be the following:
