Well, that doesn't work.
Yes, I have the full Monty of Visual Studio 2008 and MSDN installed.

To prove these do not work, tried both settings with my network cable removed and network connection DISABLED.
In both cases, the HELP system tried to open the ONLINE help - and of course failed, because there is no connection.
Here is a concrete example of it NOT working:
1) Open any of your Visual Studio .SLN files, assuming you are doing C# programming.
2) Disconnect your NETWORK.
3) highlight the word "public" in any of your .cs files.
4) press "F1"
The system tries to direct you to:
http://msdn.microsoft.com/en-us/library/yzh058ae.aspx
It fails to find it (because there is no network).
Instead you get:
ms-help://MS.VSCC.v90/dv_commoner/html/1702ec84-76f9-4a82-9043-9ab8acb8aa5e.htm
We get an "Information Not Found" in rather large bold type in the MS Document Explorer.
A side note: There are some who think you should always be connected when doing development - obviously those folks do not work while riding in trains, planes and automobiles (please smile when you read that - but it's true ;-)
There should be some setting, mysterious as it is, that searches the local hard disk or CD Help. What is that setting?

Do you have another version of Visual Studio installed. I had express version also installed. My document explorer wanted to use the express documents. Try switching the Integrated help viewer setting.
