The profile selection just makes a few tweaks to mimic what language you'll use primarily. It really has no effect that you'd likely want to revert back to later. Personally I would recommend that you use the Tools\Options to configure the settings you want. Then set up your command bars and layout the way you like. Then use Tools\Import and Export Settings to export your settings and store them in a safe spot. If you want to get back to them later (or you reinstall) then import the settings and it will put (most) everything back. The profile selection at the beginning is basically doing this for pre-defined profiles.

As for the theme you can change that via the Tools\Options -> General page. The theme shows up there. If you don't like the options then you can install the Color Theme Editor for Visual Studio extension and use one of the enhanced themes or create your own.
