As far as I know, you need to create a ribbonUI. With Orcas beta 1 you cannot customize the office menu with the designer, so you'll need to do it with XML.

 

Add a new item to your project. Select "Ribbon (XML)". In the ribbon xml you can add a officeMenu-node within the ribbon mode. From here you can create buttons, menus and more. You also get a code behind file to hook up the ribbon and your callbacks.
