I'm about to install a small deployment of AppFabric for the first time and am trying to figure out exactly how to configure it. I have three servers, two of which I'd like to use as cache hosts and the third to hold the cache configuration in a SQL DB. I believe this is referred to as the cluster management role in the docs.

Yes, it is possible. To make things simple, when installing choose to have the configuration run right after installation. Once configuring, point to your SQL instance with an account that has the right access, and while on the config wizard also make sure to choose the small cluster setup. This could also be scripted but since this is just a small cluster there may not be need to do that.
