Is there any existing software which allows the user's position in the room to control the mouse pointer? e.g. close to the camera = top of screen, far = bottom, etc.

You might be able to use the head joint on the skeleton and depth distance data to accomplish. Just modify kinect mouse to use the head joint and then you can still use the left hand to click or be creative.  Moving would be as easy as choosing that joint from the list and replacing it where the right hand joint is in the kinect mouse code.
