I was trying to get list of changesets per project using TFS warehouse - codechurn and I don't see all the list of changesets.

For example - a small project has 5 changesets but in warehouse - codechurn table it shows only 1 entry.

You mean in the [dbo].[CodeChurnView] view of your Tfs_Warehouse it lost changes for a project? And you checked that in your collection database, it exists. If so, please try to manually process the TFS data warehouse and analysis services cube. Follow the steps in this document to process your Tfs_Warhouse and Tfs_Analysis database.
