We have SharePoint 2013 in our on-premise environment

In this ,one web application is hosted.This application has Dual authentication enabled i.e. Windows User ( AD Authentication ) and FBA user ( external forms based users maintained using FBA Pack 2013 configured on SharePoint 2013)

we have designed a custom login page for authenticating the users.

FBA authentication is working fine ,however when we do windows authentication,the user authentication is working fine ,but when the user clicks on the Sign out and another user logs in,it is not logging off the user and still old user is being logged in to the system even if you enter any other user credentials also.

We need to do two factor authentication for login process ,hence this custom login page is being created.

Please advise how to fix this and sample steps/configurations required for this to work.
