I'm testing the range of features I can achieve with EM and custom fields and I wanted to create multi instances custom fields with same behavior I can achieve with the standard "Keywords" field.

For example, if  I create a custom field named "designer" which contains "A" and "B". I can't assign an item with both instances at the same time. Of course I can write "A, B" but then in the Organize tab, the item is tagged with "A, B" but the "A" and "B" tags seem to be considered as different items which is not the case with "Keywords" field.

If I understand what you are asking, custom fields don't work that way. They are intended to be single-entry fields. I use them for things like Price or submission status.
