We just noticed a few days ago, and no changes on our side to our SP farm, that the title column was missing on new records for all lists that had a title column.  We checked the column settings and saw that the title column was added to a "_hidden" group.  Changed it back to Base Column and then checked the document libraries and it still wasn't there.  We rebooted the server, checked again, still not there.  The only way to fix it is to go into each list, go into the settings, view the title column settings (no change) and then it shows up when we go to edit/add a new item.

Which is fine, there's a "fix?" But we don't want to have to go into EVERY list to do this.  And how did it get set this way in the first place.
