I know that I can right click on the heading bar and add additional fields, but I have to do that everytime I create a catalog or gallery and it is becoming a pain in the rear to keep doing it over and over.  Is there a way to save the settings for the list view so that the additional IPTC fields are always there?

You should have a button that says Save as Default at the bottom left of the custom fields dialog box. Set your custom fields, and click that.
