we want to use our Sharepoint to collaborate with our suppliers.

Do we need to configure it to be accessible from the outside (own domain), or can the app server be hosted locally in our intranet and only our Sharepoint server must be accessible from the internet?

Our fear is that if we would host the app server in our intranet, our suppliers are not able to see / use the apps we provide.

Your application server should not be accessible from the public Internet and usually you do not need that: in a SharePoint farm the application server provides services to all the SharePoint web applications and sites.
