I've configured Access Services and am trying to create a custom Access app.  I am able to create the app in SharePoint 2013, but when I try to access the site of the app, I get a http 404 error.  It creates the Access database in SQL as well.  I've removed the app and tried to create the app through Access 2013 rather than from SharePoint with the same result.

During the creation of the app, no Windows errors are generated in the event logs.  The only errors in the ULS logs that indicate any error are "Content Following" errors that seem to correspond with trying to navigate to the app's site.
