I need to be able to collect user information that is dynamic in both x and y axis, just like an excel grid. From a database I want to list students on the left (variable quantity) and subjects on the top (variable qty). In the cells I want to collect either Pass, Merit or Distinction from a drop down. On the right of every row I want a checkbox to distinguish completed row (record).  Below the form I would submission button to create/update the multiple records. So I would create multiple records in the database. If the form is re-opened I need all the information to be pre-populated.

I finally want to hook this up to a Nintex workflow that creates/updates entries in a database.
