Create a webservice with SMS sending functionality and embed the webservice into a mobile app. You cant do this with vb.net alone , but the platform specific language to create the application. e.g. Objective C in iPhone and Java in Windows Phone
VB.Net or for that matter .Net does not have any inbuilt support for sending sms.
There are many free/paid services available out there, you need to use them.
