First you need to make sure the first column in your CSV file does not start with the word "ID". If it does you may see an error along the lines of SYLK file.

To import the file into SharePoint 2013 (on-premise) go to Site Contents > Add an App > Import Spreadsheet > Select the columns including the first row with the column names. Then complete the step by importing the spreadsheet. This should create a new list along with the column names in a new SharePoint list.
