I have set up a 4 server SharePoint 'DEV' environment in Azure following a set of recommendations from a number of MS sites... Environment configuration = 1-server as AD/DC host, 1-server as SQLServer host, 1-server as SharePoint 2013 host, and 1-server as Workflow host.  I have a simple 'test' site created and can execute 'simple' list workflow events.  I have followed guidelines for creating a SMTP service to allow for email traffic to be processed, with the setup being performed on the SharePoint 2013 host.  I have tested the SMTP service successfully by creating a simple email text file and depositing it into the local C:\inetpub\mailroot\Pickup folder and following the transaction through to my corporate Outlook folder; as well I have turned on logging and observed the logs confirming that the transaction has been processed.  So I know that emails can be delivered from the SharePoint server to an external email server.
