Can anyone post a simple web application in asp.net having one form for inserting, updating, deleting records, and selecting. Also stored procedures .
Just simple application for a starter ( one layer or two layer ) is enough. Form also contains date and time control.

Otherwiae you can use this table also as shown below

OutPatient Details - Table 

OPNo - Primary Key 
Name 
Doctor_Id - Foriegn key from doctor table ( load combo box in form). 
appointmentdate
