We have a web app, that logs when it starts and stops. At the moment it is showing as running two instances, even when we have not configured scale out. Could it be that Azure has an instance running in the background for HA?

My guess is that your app was briefly running on two instances to make sure your application was fully available when an instance changed due to a platform upgrade or some other change. There is no feature out of the box that provides HA without implementing traffic manager. Does you app depend on affinity so it always hits the same instance?
