We're using Azure Hybrid Connection Manager to give our Azure Web Apps access to our on-prem SQL databases.

Everything works fine for a few weeks. Then the Web Apps fail. Investigation finds a "Disconnected" message in the Hybrid Connection Manager's UI. We fix the problem by using the Hybrid Connection Manager UI to remove the connection and then add it back. Doing so requires re-authentication with Azure to re-populate the Hybrid Connection Manager UI's list of subscriptions and, for each subscription, its list of relay services. Everything works fine again for a few weeks.

What can I do to fix this issue? It feels wrong that we add the connection (via the Hybrid Connection Manager's UI) within the context of a user signed in to the Azure portal but I don't see another way to use Hybrid Connection Manager's UI.

When your hybrid connection is set up with a running application and there is at least one HCM that has that hybrid connection configured, then the status will say Connected in the portal. If the status say it is not connected it could be either your app is down or your HCM cannot connect out to Azure on ports 80 or 443.
