There are different meaning of the expression "real time".
Please see:

There are different uses of this term which are incorrect or non-strict. If you mean anything related to a true real time system, to best of my knowledge, none of such system implements of allows installation of any form of CLR (http://en.wikipedia.org/wiki/Common_Language_Runtime[^]) and hence, C# programming is not an option for such systems.

Theoretically speaking, the situation may change, as there are different CLR-based developments beyond .NET are going on in Microsoft and third parties, but I would evaluate the likeness of emerging of some real-time system based or using CLR in near future as quite low. I would be glad to be mistaken.

Yes you should follow the three tiers architector..means Bussiness logic layer which contain ur logic and make its dll..
2. layer database layer which contain tables and storeprocedures
3. Presentation layer which is used for desgining and accessing the logic.

And latest trend you should use MVC3 which is pattern to Model view controler
