There is no a separate subject such as form design. There is nothing specific about it. You need to learn HTML in general, with CSS. You can find a lot of material on the Web. Just reading of W3 standards and trying out all different elements and styles would be enough to learn it. You can also look at some nice HTML designs, then look at the source HTML code and figure out how it is done.
