I''ve found that logging all exceptions can be quite useful.
Log4Net is a flexible library with many configurable options.

If you are working directly with the .Net data provider - test you queries and commands. If you are using MS SQL Server, Management studio can be used for this.

Assuming your application provides a GUI that allows the user to alter configuration settings: Log all configuration changes made by the user to a log.

Hopefully this will provide you with enough information when something goes wrong to accurately determine why, or at least have some notion about where to start.
